Wednesday October 7, 2026
A cross-border collaboration opens the Summit, as the Healthspan Action Coalition and the Cure Alliance join leading Italian organisations including the Bicocca Foundation, with Camillo Ricordi, MD and Giovanna Iannantuoni, PhD among the voices. David Barzilai, MD, PhD, DipABLM gives the day’s keynote on turning the age wave into opportunity, and patient-advocacy voices including Sabrina Cohen and Anne Marie Miller close the day.

The day’s headline moment is “The Ultimate Longevity Fireside Chat”, bringing together Aubrey de Grey and Antonio Regalado of MIT Technology Review. Camillo Ricordi, MD opens with a keynote on healthspan engineering to prevent the collapse of reactive health systems, and Anita Goel, MD, PhD explores translating biomarkers of aging into precision longevity medicine — measuring the individual over time.

HRH Princess Dr Haya Bint Khaled Bin Bandar Al Saud of Hevolution gives the day’s keynote. A policy panel, “Moving the Needle in Washington”, brings together Randall Rutta of the National Health Council with G. Alexander Fleming, MD and Thomas Seoh of Kitalys, and a further panel examines right-to-try laws in the United States, chaired by Carolyn Baker Ringel.

Steven A. Garan, PhD leads a panel on women’s health, hormones and healthspan alongside HRH Princess Dr Haya Bint Khaled Bin Bandar Al Saud. Shelly Jordan, MD — a pioneer in regenerative neurology, advanced neuroimaging and brain rejuvenation — delivers a keynote, and a panel on addiction and healthspan is chaired by Marion J. Riggs, PhD.

Measuring the vessel, not the guess
Longevity medicine has a measurement problem it is honest about: most of what we call biological age is inferred from proxies, and most interventions are judged by whether the proxy moves. The microcirculation offers something less indirect. The endothelial glycocalyx — the gel-like layer lining every blood vessel — can be imaged under the tongue, in the clinic, in about five minutes, without a needle.
Hans will set out what the measurement is, what the published literature does and does not support, and what happens when you re-measure the same person after an intervention. Including the cases where the vascular measure disagrees with the metabolic one.

Why we are at this Summit
What a translational audience should ask us
This is a meeting about moving things from the bench to the bedside, which means the interesting question about any measurement is not whether it is impressive but what it does and does not support. Five findings from the published literature, including the one that cuts against us.
Vascular aging is measurable, not inferredTrack: Biomarkers & Outcomes
Comparing young and old mice and younger and older adults, glycocalyx thickness in sublingual vessels was 33% lower in the older human participants, with the perfused boundary region 10–22% greater and perfused microvascular density 16–21% lower. The same pattern held across both species. Age shows up in the microcirculation as a number you can take at the chairside.

When the scale improves and the vessel does notTrack: Clinical Implementation
In a three-arm randomised controlled trial in 56 adults with type 2 diabetes, a monthly five-day fasting-mimicking diet lowered BMI by 1.0 kg/m² and HbA1c by about 5 mmol/mol — while the dynamic perfused boundary region got 0.32 µm worse, and stayed worse at six-month follow-up. A glycocalyx-targeted supplement arm (Endocalyx, not one of our products) moved the same measure 0.73 µm the other way, with a 0.7-point rise in the microvascular health score. Two interventions, two verdicts, and the metabolic reading alone would have told you the wrong story about one of them.
Re-measurement tracks a real trajectoryTrack: Clinical Implementation
66 treatment-naive adults were measured before starting antiretroviral therapy and again at 24 and 48 weeks. The perfused boundary region fell progressively — 2.17 µm to 2.04 to 1.93 — improving in around two-thirds of individuals, and yet still sat above published healthy-control values at a year. Partial recovery, visible as partial recovery. That is what a biomarker is for.

What the measurement is notTrack: Biomarkers & Outcomes
In the largest study of its kind — 6,169 multi-ethnic adults — a single sublingual reading was independently associated with diabetes, older age and female sex, but was not associated with prevalent cardiovascular disease, coronary heart disease or stroke. A one-off number is not a cardiovascular risk score, and we do not present it as one. Its value is in what it does over time, in the same person, either side of an intervention.

Where the number comes fromMethodology
In 915 middle-aged adults from a population cohort, how deeply red blood cells penetrate the glycocalyx explained about 47% of the variance in perfused boundary region, and tracked inversely with how much of the time a vessel segment actually contained red cells. The measurement is derived from red blood cell dynamics in your own capillaries, not estimated from a questionnaire.

About the Translational Longevity Summit
The Translational Longevity Summit is produced under the Boston BioLife umbrella with the Healthspan Action Coalition, the World Stem Cell Summit and the Regenerative Medicine Foundation, and it runs at 1 Milestone Place on Harvard’s Enterprise Research Campus from 7 to 11 October. Six scientific tracks, more than seventy-five speakers, and a format built so you can take one day, three, or all five.
Its stated frame is the gap between discovery and deployment: what actually reaches a patient. That makes it an unusually demanding room for anyone selling a measurement, which is precisely why we took the Premier level — it comes with a session on the programme rather than a louder booth.
